In the almost 13 years that we have owned our house, we refinanced twice.  In both cases there was no transfer of assignments to the new lender just new deed of trusts were registered.  Is this how it is done?   A friend told me that there should have been deed of reconveyance done in both cases.  Is he right?  If he is correct, would this be considered a break in chain of title?

your friend is right. there needs to be a relaese on file to show that the previous loans have been paid off.
